KnKt’d Privacy Agreement

HIPAA is the federal Health Insurance Portability and Accountability Act of 1996. The primary goal of the law is to make it easier for people to keep health insurance, protect the confidentiality and security of healthcare information and help the healthcare industry control administrative costs.

Anytime the name KnKt’d or Synergistic Creations is used singularly or together all provisions of these agreements apply to include all employees, affiliates, and subcontractors.

Synergistic Creations (owners of KnKt’d platform) has developed the following privacy policy to protect individually identifiable information. This policy covers Synergistic Creations (owners of KnKt’d platform) and applies to all individually identifiable information that Synergistic Creations (owners of KnKt’d platform) obtains when a client and end user uses the KnKt’d platform (owned and operated by Synergistic Creations).

Your privacy is of the utmost importance to us and it is the law. The following are areas of importance about your privacy and the security of your information.

Know that when you register and or click acceptance of the Terms of Use for the KnKt’d platform service you are agreeing to and have read both the Terms of Use and Security and Privacy Statements.

Synergistic Creations (owners of KnKt’d platform) will not sell, trade, or disclose to third parties any individually identifiable information, derived from the use of any Synergistic Creations (owners of KnKt’d platform) service, (except as required by subpoena, search warrant or other legal process or in the case of imminent physical harm to the customer or others). When Synergistic Creations (owners of KnKt’d platform) uses other agents, contractors or companies to perform services on its behalf, Synergistic Creations (owners of KnKt’d platform) will ensure that the company protects the individually identifiable information consistent with this policy.

Synergistic Creations (owners of KnKt’d platform) has implemented technology and security features and strict policy guidelines to safeguard the privacy of identifiable information from unauthorized access or improper use, and we will continue to enhance our security procedures as new technology becomes available.

The company will not read or disclose to third parties private e-mail communications that are transmitted using Synergistic Creations (owners of KnKt’d platform) services except as required to operate the service or as otherwise authorized by law.

Synergistic Creations (owners of KnKt’d platform) may use individually identifiable information to investigate and help prevent potentially unlawful activity or activity that threatens the network or otherwise violates the client agreement for that service.

Synergistic Creations (owners of KnKt’d platform) honors requests from clients for changes in account information, such as name and password and will correct any such information that is inaccurate.

Synergistic Creations (owners of KnKt’d platform) will not knowingly disclose an individual's personal information to third parties, except when one or more of the following conditions is true:

• We have the individual's permission to make the disclosure
• The disclosure is required by law or professional standards
• The information in question is publicly available
• The disclosure is reasonably necessary for the establishment or defense of legal claims

Synergistic Creations (owners of KnKt’d platform) takes data security seriously and has put in place appropriate physical, electronic, and procedures to safeguard and secure the information we process.

Synergistic Creations (owners of KnKt’d platform) will use personal information only in ways that are compatible with the purposes for which it was collected, which is in the confines of e-discovery usage.

Upon request, Synergistic Creations (owners of KnKt’d platform) will grant individuals reasonable access to personal information that it holds about them. In addition, Synergistic Creations (owners of KnKt’d platform) will take reasonable steps to permit individuals to correct, amend, or delete information that is demonstrated to be inaccurate or incomplete.

Synergistic Creations (owners of KnKt’d platform) utilizes the self-assessment approach to assure its compliance with our privacy statement. Synergistic Creations (owners of KnKt’d platform) periodically verifies that the policy is accurate, comprehensive for the information intended to be covered, prominently displayed, completely implemented, and in conformity with the HIPAA Privacy and Security Principles.

Synergistic Creations in the future may request providers and their end user to opt into research studies on treatment modalities and or pharmaceuticals. If and when this is the case, you will opt into the research study providing the required data. All identifiable information for the end users will be removed for these studies.

All sensitive data will be encrypted at rest and in motion. Only the provider and the provider’s end user have access to this information due to the nature of encryption. Unless release if required by law by nature of warrant. Regular information will be aggregated utilizing the cloud database and stored there. Sensitive information will be stored on a hard server under layered encryption.

Synergistic Creations (owners of KnKt’d platform) will store data in compliance with medical record laws. This information will be stored for 10 years or as state and federal law dictates.

If your account becomes inactive the data will be stored for the period of 10 years and as aforementioned in the previous paragraph.

Synergistic Creations (owners of KnKt’d platform) will keep this policy current and will inform you of any changes that we make.
